Create an Eventlink Account to participate in Athletics - Required paperwork

How to Create an Account on Eventlink and start filling out forms to participate in WT Middle School athletics

1. Go to www.eventlink.com and click create account 


2. Search for and add + (add) your school 


3. Complete the form with parent’s information


4. Be sure to click add contact – add both your email and phone number to get texts and emails


5. Complete registration 

 

Click the  Eventlink Registration tab 

Add your student + (add) and save

Click on the Eventlink Registrations and click on the Athlete Registration

Select the items you wish to complete 

            If you played last year and have a physical on file dated after 4/1/19

Complete form 1a - IHSAA Health History Update 

                        1b - Franciscan Sports Medicine Authorization

Fall Sport: Choose Your Sport 

Sign Protect Washington Township pledge

If you do not have a physical on file complete the above paperwork as well as

Complete form 1c - IHSAA Physical (PPE) 

                        1 d - Signature form for Physician to sign 

Subscribe to School Calendars

1. Click on “My Calendars” 


2. Select “Manage My Calendars” from the drop-down 


3. Add + (add) the calendars you want to follow and SAVE Pick your child's team
